Access the NC COVID-19 Vaccine Portal

If you got a COVID-19 vaccine or booster in North Carolina, your vaccination information may be online in the COVID-19 Vaccine Portal. How you access it depends on where you got the vaccine:

If you got your vaccine here... ...get your information here:

A pharmacy or supermarket, like:

  • CVS, Walgreens, or Walmart
  • Harris Teeter, Publix, or Food Lion
  • Costco or Sam's Club
  • Other pharmacies or grocery stores

You must call us to activate your CVMS account:

8 a.m. to 4 p.m. Monday-Friday

  • A doctor's office or hospital
  • A local health department or community event 
  • A FEMA or local vaccination drive, including at a church or school 

A COVID-19 Vaccine Portal account has been set up for you, but you will need to reset the username and password. 


Use the section “If You Don’t Know Your Username”

  • A military base
  • The Department of Defense
  • The Department of Veterans' Affairs (The VA)
  • At a Tribal or Urban Indian Health facility
  • Cherokee Health Systems
  • Indian Health Service
  • Outside North Carolina
We’re sorry, but the COVID-19 Vaccine Portal doesn’t have your vaccine information. You will need to talk to the place where you got your vaccine or booster.

You don't need your vaccine information to get an updated vaccine.

If you don't have your vaccine card, you'll be given a new one. Don't wait: find an updated vaccine location near you.
