Help: Using the North Carolina COVID-19 Vaccine Portal

If you received your COVID-19 vaccine or booster in North Carolina from a pharmacy, grocery store, doctor's office, hospital, health department, or community event, your vaccination information may be in the COVID-19 Vaccine Portal.

How to use the COVID-19 Vaccine Portal 

What is the COVID-19 Vaccine Portal?

North Carolinians can log in to the COVID-19 Vaccine Portal to get their COVID-19 vaccination information. A printed or downloaded version of this information should, in most cases, serve as valid proof of COVID-19 vaccination status. (See more about proving your vaccination status.) 

The COVID-19 Vaccine Portal does not have information about other vaccinations or immunizations (like flu vaccines, for instance), or any other medical or health records. 

How do I register for an account?

You are automatically registered in the COVID-19 Vaccine Portal if you received your vaccine or booster from a North Carolina pharmacy, grocery store, doctor's office, hospital, health department, or community event.

However, you will need to activate your account and set a password before you can access the system. If your e-mail address is on file, you should have received instructions in your inbox. (Check for a message from nccvms@dhhs.nc.gov.) 

If you did not receive an email, use our "Find Your Username" service on the COVID-19 Vaccine Portal log in page. If your email address is not found, you will need to activate your account by phone. Contact the NC COVID-19 Vaccine Help Center at (888) 675-4567 to request access to your account (7 a.m. to 7 p.m. Monday to Friday, 8 a.m. to 4 p.m. Saturday & Sunday).

How do I log in?

To log into the portal, you’ll need your username and password. Your username is generated automatically by the system and is not the same as your email address.

If your e-mail address is on file, you should have received an email from nccvms@dhhs.nc.gov with your username and a link to create your password.

If you did not get this email or can’t find your username, use our "Find Your Username" service on the COVID-19 Vaccine Portal log in page. If your email address is not found, you will need to activate your account by phone. Contact the NC COVID-19 Vaccine Help Center at (888) 675-4567 to request access to your account (7 a.m. to 7 p.m. Monday to Friday, 8 a.m. to 4 p.m. Saturday & Sunday).

Once you know the account's username, log in to the Portal. If this is your first time logging in, you may need to set or reset the account's password first.

What if I can’t remember my username or password?

To retrieve your username, use the "Find Your Username" service on the COVID-19 Vaccine Portal log in page. An email will be sent to you with further instructions.

If your email address is not found, you will need to activate your account by phone. Contact the NC COVID-19 Vaccine Help Center at (888) 675-4567 to request access to your account (7 a.m. to 7 p.m. Monday to Friday, 8 a.m. to 4 p.m. Saturday & Sunday).

If you know your username, you may reset your password.

 
How do I view and print my vaccination information document?

Once logged in, select the blue "GENERATE YOUR COVID-19 VACCINE INFORMATION" button. (Pictured below is the view on a mobile phone.) If you do not see the button on the home screen, this means that CVMS does not have any COVID-19 vaccinations on record for you.

A PDF document with your vaccine information will download to your device or open in another tab of your browser. Use the "Download" feature of your browser, if there is one, to save a copy, or the "Print" feature, if there is one, to print a copy.
