How to View Your COVID-19 Vaccination Information Version en Español Many North Carolinians can access their COVID-19 vaccine information in the North Carolina COVID-19 Vaccine Portal, including anyone who: Received their COVID-19 vaccine from a North Carolina provider, AND Provided an email address to a North Carolina vaccine provider If you received your vaccine from a pharmacy participating in the Federal Retail Pharmacy Program or from another federal vaccine provider such as the U.S. Department of Defense, you will need to get your vaccine information directly from that provider. It is not available in the North Carolina COVID-19 Vaccine Portal. Download Print-Friendly Flyer Step 1. Go to the North Carolina COVID-19 Vaccine Portal Go to https://covid-vaccine-portal.ncdhhs.gov click "Login" in the top right corner. Be sure you are using the latest version of your web browser. The COVID-19 Vaccine Portal does not work in the Internet Explorer or older Microsoft Edge browsers. To change the language to Spanish, click the "Preferred Language" bar. Step 2: Login to the Portal Enter your username and password. If you forgot your username and/or password, click the “Forgot your password?” and/or “Forgot your username?” links on the login page. Follow the prompts. After logging in, the COVID-19 Vaccine Portal page appears. Step 3: Go to My Dashboard tab On the COVID-19 Vaccine Portal page, click ‘My Dashboard’ at the top of the screen. If you have not yet received your first COVID-19 vaccine dose, there will not be any information displayed. Please note that it may take a few days after you receive your first dose for the information to appear in the system. Step 4: Go to Your COVID-19 Vaccine Information On the ‘My Dashboard’ page, click ‘COVID-19 Vaccine Information’ to view your information. Step 5: Download or Print Your Vaccine Information Select the “Generate Vaccine Information PDF” button. This will appear on the right-hand side or at the bottom of the page. The PDF will either download to your device or open in another tab of your browser. If it opens in another tab, use the Download feature of your browser to save a copy or the Print feature to print a copy, typically found in the top right of browsers. The PDF includes a QR code that can be scanned by a phone or QR scanner. When scanned, the QR code pulls up a person’s vaccination information. The information pulled from the QR code contains confidential health information and is intended for the vaccine recipient’s use only. Note: The QR code provides the information in English only. Step 6: Use the QR Code How to Use the QR Code - Apple Open your camera and scan the QR code. Swipe down on the notification from Safari. (Do not click on the notification. Clicking on the link will open up a web search.) Once you swipe down, the vaccination information will appear on the phone as shown below. You can take a screenshot of the information or press the “Copy” button to paste the application into another app. How to Use the QR Code - Android Open your camera and point it at the QR code to scan. Tap the magnifying glass icon to scan the QR code. Tap the pop-up notification. The vaccine information will appear on the phone as shown below. Need additional help?
